Steps & Forms
PLEASE NOTE:
These steps are relevant to the initial home study process, and may not be the same for post-placement reports or home study updates. This list may not fully complete your home study process. These tasks need not necessarily be completed in the order listed. Please check with Claire and/or your attorney to make sure that you have completed all necessary client responsibilities. Responsibilities may be different for a relative or "known" child adoption.
1. Contact Claire to set up a free initial appointment at her office (if you are able to travel to the North Seattle area), or in your home.
2. Download (in PDF format) and complete the following forms (if you are unable to download these forms, they will be provided to you at your first appointment):
> Registration form (to be submitted with non-refundable $200 enrollment fee at first visit)
> Autobiography guidelines **
> Parent medical form
> Child medical form **
> Family financial statement
> Reference letter request **
(A minimum of four reference letters are requested per applicant family. Please do your best to cover these
categories: family member, friend, neighbor/neighborhood resident, and co-worker.)
> Authorization to perform background checks (WA)
> Background check information sheet
3. Complete and submit your autobiography. Autobiography can be submitted by e-mail, fax, regular mail or in person.
4. Begin gathering and preparing the following documents:
> Doctor's forms
> Financial statements
> Reference letters **
> Authorization to perform background checks (WA)
5. Schedule and complete your home visit, as required by Washington State regulations.
6. Submit all outstanding documents and payment.
7. Review and approve home study.
8. You're done!
